Rationale


The past few years have seen considerable investment in chronic disease management in primary care, particularly relating to diabetes. This has included new forms of payment for GPs and practices, payments for allied health services and subsidies for practice nurses in rural and outer urban areas, support programs through Divisions, support for self management and developments in information systems. It is important that these developments take full advantage of relevant evidence of effective strategies

Aims


To identify evidence based interventions in diabetes prevention, detection, management and consumer support
  • To map the evidence against the various national policies and programs for diabetes
  • To identify strategies to increase integration between general practice, primary health care services, NGOs and between Commonwealth funded and State funded programs.

    Literature review

Identification of current and planned national policies and programs for diabetes

Consultation with key informants to identify strategies to increase integration
