NSW Health is seeking to develop performance indicators to support implementation of its Primary and Community Health Policy.
Aims
To develop an agreed set of core indicators to be considered for inclusion in the Service Agreements for Area Health Service Chief Executives that reflect the Integrated Primary and Community Health Policy.

Issues paper, workshops with key stakeholders and final report.
